Then, you will practice with a case study (see directions below).<br>Case Study History Practice <br><br>Choose who will be the patient and provider first. Patient gets a case study, provider gets a history form. Conduct and document a professional interview. The first slide should include the disease name, the patient name, and your name. The last slide should include 3 sources in alphabetical order. Include throughout your presentation where you used each source.  <br>Make sure this is written in "patient friendly" terminology. Do not use terminology that your patient might not understand.
